Geneva sits at the north end of Seneca Lake in the Finger Lakes, a small city with a college, a hospital and a farming and wine economy around it. We see adults across Ontario and Seneca counties by secure video.
At the head of Seneca Lake
Geneva sits at the northern end of Seneca Lake, the deepest of the Finger Lakes, which runs thirty-eight miles south from here toward Watkins Glen. The city is unusual administratively: it lies in both Ontario and Seneca counties, though every inhabited part of it is in Ontario County and the Seneca County portion is entirely water.
It is a city of about 12,800, home to Hobart and William Smith Colleges, a hospital, and the agricultural research station that anchors a good deal of the region’s farming science. The Finger Lakes is the largest wine-producing region in New York State, and Geneva sits at the center of it.
A tourism and agriculture economy, and what it does to a year
This is the most useful thing to understand about mental health here, and it is genuinely different from the post-industrial pattern common further west.
Geneva’s economy runs on agriculture, wine and tourism, plus the college and the hospital. The first three are seasonal in a way an office job is not. There is a compressed stretch from late spring through harvest where the working days are extremely long, followed by a much quieter winter, and for a lot of households the income across those two periods is not remotely comparable.
Two clinical consequences that we take seriously rather than mention in passing:
Anxiety tied to unpredictable income is a specific presentation. It is not the same as anxiety about a demanding but stable job, and it does not respond well to generic reassurance, because the underlying uncertainty is real. What helps is structured work on the response and on what is genuinely controllable, which is what CBT is built to do.
The off-season is when things surface. During the season people are too busy to notice how they are doing. In January, with the work gone quiet and the daylight short, it arrives all at once. That timing is predictable enough to plan treatment around.

Agricultural work is not a metaphor here
A brief note, because it matters for how the work is done.
Farming and vineyard work involve physical labour, financial exposure to weather, long isolated hours and a set of pressures that a therapist unfamiliar with them can easily misread as avoidance or poor self-care. Someone who cannot attend a Tuesday afternoon appointment in September is not resisting treatment; they are harvesting.
A plan that ignores that fails, and the failure gets attributed to the client. Sessions at the edges of the day, a schedule that flexes across the season, and goals that fit the actual year are not accommodations. They are the difference between therapy that works here and therapy that gets abandoned in week five.
How care is structured
Four stages, in a set order.
The intake opens the work and ends with a 0 to 10 intensity rating that becomes your baseline. The history session follows, covering history, health, relationships and stressors, with any question declinable. The treatment plan is third, written with you in your own language, including a goal you nominate. If your year is seasonal, this is the stage to say so.
Then weekly sessions and a monthly review against standardized measures. Your therapist shows you the trend directly, and a flat one revises the plan rather than the expectations on you.
CBT is the main modality: structured, present-focused, skills-based.
Video, stated carefully
For the outpatient concerns in our scope, the research comparing video CBT with in-person CBT has generally reported comparable outcomes. That is the whole of the claim and we will not inflate it.
Where a person needs more than weekly outpatient sessions can offer, we say so and help arrange the right level of care rather than keeping them on the schedule.

- The wider picture
Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.
- Building the plan
You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.
- Ongoing
Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.
Therapy here is measured, not guessed
Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
