Hempstead is the most populous village in New York State. It sits in the middle of one of the wealthiest counties in the country, and the availability of outpatient therapists who take commercial insurance here does not remotely track that.

The pattern people describe is consistent. The practices with openings are private-pay. The ones that take your card are booked. Transportation adds a second filter on top: if getting to an appointment means a bus, a transfer and an hour each way, a weekly commitment is not realistic no matter how motivated you are.

The early work fills in the wider context of your life, at your pace, and turns into a written treatment plan made with you rather than handed to you, carrying a personal goal alongside the clinical ones. The treatment plan is written with you, in your own phrasing, and includes a goal that belongs to you rather than to a diagnosis.

After that, weekly sessions, with a monthly review using standardized measures. Your therapist shows you the trend. If it is not moving, the plan changes rather than you being told to try harder.

Our primary approach is CBT, which is structured, present-focused and skills-based. Where a different approach fits better, your therapist will name it.

On video sessions

For the outpatient concerns we treat, research comparing video-delivered CBT with in-person CBT has generally found comparable outcomes. That is the accurate, limited version of the claim.

A practical point: a private space is not a given in a full household. Clients take sessions from a parked car, a break room, or a bedroom with headphones and a closed door. Tell your therapist the constraint and it gets planned around.
