Gloversville is the largest city in Fulton County and half of the Glove Cities, a market where the industry that named the place is gone and the local mental health capacity is limited. We see adults across Fulton County by secure video.

Named for an industry that no longer exists

Gloversville is the largest city in Fulton County, in the Mohawk Valley at the southern edge of the Adirondack foothills. The name is exactly what it sounds like.

This city and neighbouring Johnstown were the center of the American glove industry for about ninety years, with more than two hundred glovemaking manufacturers operating in Gloversville alone at its peak. Together the two are still called the Glove Cities.

Foreign competition ended it. The decline began in the 1950s and accelerated sharply through the 1980s and 1990s. The city’s population, once far higher, is about 15,100 now, and the city has spent decades working through the consequences: deindustrialization, population loss, poverty and the problems that follow those.

We state that plainly because it is documented, and because a page about mental health here that pretended otherwise would be useless.

What that means for the people we see

Two things, and it is worth separating them.

The first is the generational pattern common to post-industrial small cities: depression that has been present in a family and a place long enough to be mistaken for temperament. It is not temperament, and it is treatable.

The second is more practical. A county that has been losing population and tax base for decades does not build up a large outpatient mental health sector. Fulton County’s local capacity is genuinely limited, and the realistic alternatives involve driving to Amsterdam, Schenectady or Albany, which is a real trip from here and a worse one between November and March.

Behavioral activation has to fit the county

A clinical note rather than a logistical one, because it changes how the work is actually done here.

Behavioral activation is a core CBT technique for depression: deliberately rebuilding contact with activities that reliably lift mood. Standard descriptions of it quietly assume an urban environment with many options close at hand.

In Fulton County in February, a plan built on that assumption collapses in the first week. Your therapist will build one around what is genuinely reachable here, which takes more thought and works considerably better than a generic worksheet.

How care is structured

Four stages, in order, no variation.

Intake first, finishing with a 0 to 10 rating of intensity that sets the baseline. Your history second, across history, health, relationships and stressors, with everything optional. Treatment plan third, written with you in your own phrasing, including a personal goal you choose.

Then weekly sessions, with a monthly review against standardized measures. The trend is shown to you. Flat means the plan gets revised.

CBT is the core approach: structured, present-focused, skills-based. As noted above, the behavioral activation element gets built from what is actually reachable in this county rather than from a generic list.

Video, stated plainly

About whether this works: for the outpatient presentations we treat, research putting video-delivered CBT alongside the in-person form has generally found comparable outcomes. We will not oversell that.
