Johnstown is the Fulton County seat, which puts the courts, the county offices and the county's administrative business in a city of about 8,200 people. We work with adults here by secure video and can usually start within days.

The county seat, three miles from its twin

Johnstown is the seat of Fulton County, in the Mohawk Valley about forty-five miles northwest of Albany, in the foothills of the Adirondacks. It was named for Sir William Johnson, its colonial founder, and it holds one of the older continuously used courthouses in the state.

It is also three miles from Gloversville, its larger neighbour, and the two are known together as the Glove Cities. Anyone in this county will tell you they are distinct places with a long rivalry, and they are right, but for most practical purposes they function as one small urban area of about 23,000 people.

What genuinely separates Johnstown is what a county seat does: the courts, the county offices, the DA, probation, family court and social services are all here, in a city of 8,200.

What a courthouse city means for privacy

This is the sharpest and most specific thing about seeking therapy in Johnstown, and it is the reason this page emphasises it.

In a small county seat, a large share of the adult population has some business at the county complex at some point: jury duty, a family court matter, a filing, a job. Downtown Johnstown is where people run into each other, and the streets near the courthouse are the most observed few blocks in the county.

Walking into a counseling office in that specific area is, for a lot of people, simply not going to happen. That is not paranoia. It is an accurate assessment of a small city where the same few hundred people circulate through the same few blocks, and it stops people from getting care they need.

A video session from your own house removes the entire problem. No parking downtown, no waiting room, no chance of sitting across from someone you know. In this city that is not a convenience feature. It is frequently the deciding factor.

What we do not do

Because this is a courthouse city, it is worth being explicit about the boundary, and we would rather say it here than after someone books.

We provide treatment. We do not provide court-ordered evaluations, forensic assessments, custody recommendations, fitness-for-duty opinions or expert testimony, and we do not write letters advocating for a legal outcome.

Those are a different professional role, and mixing them with treatment damages both. If a court has ordered an evaluation, you need a forensic evaluator, not a treating therapist. If you are in treatment with us and a legal matter arises, we will tell you clearly where the line is.

Therapy itself is confidential, with the narrow legal exceptions that apply everywhere, principally an imminent risk to someone’s safety.

Anxiety is the most common presentation, usually attached to specific circumstances rather than free-floating: employment, money, a legal or family matter, health, an aging parent.

A specific observation about this market rather than a general one.

When your life includes an open legal matter, a custody schedule or a probation requirement, your stress runs on a calendar you do not control. Adjournments move. Hearings get rescheduled. The date you have been bracing for slides by six weeks and the bracing simply continues.

That produces a distinct pattern of anxiety, and it is not well served by advice about controlling what you can control, because the honest answer is that a great deal of it is genuinely outside your control. What does help is structured work on the response rather than the circumstance, which is what CBT is actually built for.

How care is structured

Four stages, in a fixed order.

The intake comes first and ends with a 0 to 10 intensity rating that becomes your baseline. The history session is second, spanning history, health, relationships and stressors, and any question can be declined. The treatment plan is third, written jointly, in your own words, with one goal you choose yourself.

Then weekly sessions and a monthly review using standardized measures. Your therapist shows you the trend line directly. If it is flat, the plan changes.

CBT is the main modality: structured, present-focused, skills-based. It is a good fit for stress that runs on an external calendar, because it works on the response rather than pretending you control the schedule.

Video, stated plainly

Comparisons between video-delivered CBT and in-person CBT have generally found similar outcomes for the outpatient concerns we treat. Stated exactly that narrowly, because overselling it would be dishonest.

    Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.

  2. The wider picture

    Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.

  3. Building the plan

    You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Therapy here is measured, not guessed

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.

Sessions are weekly for the first two months to build a foundation, then frequency is reassessed with you. You set the pace, and you share only what you are comfortable sharing.
