Lockport is the Niagara County seat, about twenty-five miles from our Buffalo office, and it serves a wide rural county from a small downtown. Local caseloads fill quickly as a result. We work with adults here by secure video and take the Western New York plans people in this county carry.
The city the canal had to climb
Lockport exists because of an engineering problem. The Erie Canal had to get barges up and over the Niagara Escarpment, a sixty-foot rock ridge, and the answer was a stacked set of locks cut into stone. The city took its name from them, and the original Flight of Five is still there in Lowertown.
It is now the seat of Niagara County, a small city of about 21,000 that provides the county courts, offices and most professional services for a largely rural county stretching north to Lake Ontario.
A county-seat bottleneck
This is the practical reason people here end up on a waitlist.

The problem is that a course of therapy is not one trip. It is twenty of them, and roughly eight of those fall between December and March in a county that sits in the lake-effect belt off Lake Ontario. Add a rural drive in from Newfane or Gasport before the highway, and the failure mode is predictable: the appointment gets rescheduled, then rescheduled again, then quietly dropped.
A session that starts at your own table has none of those failure points. In this county continuity is the strongest practical argument for how we work.
How care is structured
Four stages, run in order, and none of it is improvised.
The intake opens things, finishing with a 0 to 10 rating of intensity that fixes your starting point. The history session follows, covering history, health, relationships and the pressures currently on you, and nothing in it is compulsory to answer. The treatment plan is third, drafted with you rather than handed to you, in language you would actually use, with a personal goal you set.
After that, weekly sessions and a monthly review using standardized measures. Your therapist puts the trend in front of you. A trend that will not move is a signal to change the plan.
The primary approach is cognitive behavioral therapy, structured and present-focused. Where another method suits you better, your therapist raises it.
Video, stated plainly
For the outpatient concerns we work with, head-to-head research on CBT by video versus CBT in person has generally reported comparable outcomes. Deliberately narrow wording, because that is what the research supports.

- Ongoing
Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

My internet is not great outside the city. Is that a problem?
Often manageable. Video needs a stable connection more than a fast one, and a phone on cellular data sometimes works better than rural home service. If it keeps interrupting sessions, say so and your therapist will work through the logistics with you.
