Salamanca is a small Cattaraugus County city on the Allegheny River, roughly sixty miles from our Buffalo office. Local outpatient capacity is limited and the drive to anything else is long. We work with adults here by secure video and take the Western New York plans that sell in this county.

Salamanca sits on the Allegheny River in southern Cattaraugus County, surrounded by hill country and bordered by Allegany State Park, the largest state park in New York. It is a city of about 5,900, one of the smallest in the state, and it grew as a railroad junction in the nineteenth century.
Salamanca is also located within the Allegany Territory of the Seneca Nation of Indians, which is a genuinely unusual arrangement among American cities and shapes local government, land tenure and daily life here in ways that have no parallel in most of the state.

Depression is the leading presentation, and in a small rural city with a long economic contraction behind it that is not surprising. What we see most is not acute crisis. It is a persistent flatness that has been present long enough to feel like a personality trait rather than a condition.
It is treatable, and that is worth saying directly.
Anxiety is close behind, usually attached to something concrete: employment, money, health, an aging parent, distance from services.
Grief is a substantial share in a community where families stay put across generations and losses accumulate in the same place.

Those are different problems and they respond to different interventions.

The main modality is CBT, structured and present-focused, and in a rural county the between-session skills work matters more than usual, because it is what carries the work through a week you spend largely on your own.
