Port Jervis sits where New York, New Jersey and Pennsylvania meet, at the far western corner of Orange County. It is a small city a long way from anything, and the tri-state position creates a licensure question worth understanding before you book. We see adults here by secure video.

Where three states meet

Port Jervis is a small city of about 8,800 at the extreme western tip of Orange County, on the Delaware River, at the point where New York, New Jersey and Pennsylvania meet. The spot is literally marked; the neighborhood next to it is called Tri-States.

It grew as a canal and then a railroad town, named for the engineer of the Delaware and Hudson Canal, and it is still the end of the line on Metro-North’s Port Jervis branch.

For most cities the geography section is context. Here it is a practical constraint on whether we can work with you at all, so it goes near the top.

A therapist must be licensed in the state where you are physically sitting during a session. This is a licensure rule, not a preference, and it does not bend for proximity. Our clinicians are licensed in New York.

In most of the state that is invisible. In Port Jervis it is not, because thousands of people here cross a state line as part of an ordinary day. Matamoras, Pennsylvania is directly across the river. Montague, New Jersey is minutes away. People live on one side and work, shop and see doctors on another.

What that means concretely:

  • If you live in New York and your session happens at home, we are fine, wherever you work.
  • If you live in Pennsylvania or New Jersey, we cannot see you at home, even though you may think of Port Jervis as your city.
  • If you live in New York but will be at work across the line at session time, that session needs to be rescheduled rather than taken from the wrong side of the river.

We would rather explain this before you book than cancel afterward.

A long way from everything

Port Jervis is about eighty miles from our Queens office and a similar distance from most of the larger practices in the Hudson Valley. Middletown, the nearest place with meaningful outpatient capacity, is roughly twenty-five miles east.

For a weekly fifty-minute appointment, a fifty-mile round trip to Middletown is the realistic local alternative, and that is the version people try before they call us. It works in September. It works less well in February, and it works badly once anything else in a week goes wrong.

Video removes the drive entirely, which in this corner of the county is the difference between a course of therapy and a good intention.

What people come in for here

Depression is the leading presentation in this market. Port Jervis has been through the long decline of the railroad economy that built it, and much of what we see is a persistent low mood that has been present long enough to be mistaken for temperament.

Anxiety is close behind, generally anchored to something concrete: employment, money, health, an aging parent, or the compounding logistics of living far from services.

Trauma is a meaningful share and responds to structured, evidence-based treatment.

Grief comes up steadily, frequently layered and often old.

Distance shapes the treatment plan, not just the logistics

This is a real clinical point rather than a scheduling one.

Behavioral activation is a core CBT technique for depression: rebuilding contact with activities that reliably improve mood. Most descriptions of it quietly assume a city, where the options are numerous and close.

In a small city ringed by a lot of empty country, a plan built on that assumption fails in week two. Your therapist will build one around what is actually within reach here, which is a different exercise and one worth doing properly rather than generically.

How care is structured

Four stages, in a fixed order.

The intake is first, ending with a 0 to 10 intensity rating that becomes your baseline. The history session is second, spanning history, health, relationships and stressors, with any question declinable. The treatment plan is third, written with you in your own words, including a personal goal you choose.

Then weekly sessions, reviewed monthly against standardized measures. Your therapist shows you the trend. A flat line changes the plan.

CBT is the main approach, structured and present-focused. As noted above, the behavioral parts of it get built around what is genuinely reachable from here rather than from a generic template.

Video, stated plainly

For the outpatient concerns we work with, head-to-head research on CBT by video versus CBT in person has generally reported comparable outcomes. Deliberately narrow wording, because that is what the research supports.

What does therapy here actually look like?

The first three sessions follow a clear structure, so you always know what is coming next.

  1. The first meeting

    Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.

  2. The wider picture

    Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.

  3. Building the plan

    You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Therapy here is measured, not guessed

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.

Sessions are weekly for the first two months to build a foundation, then frequency is reassessed with you. You set the pace, and you share only what you are comfortable sharing.

Yes. Medicare is one of the plans we are in-network with for New York telehealth. We confirm your specific coverage, including any secondary plan, before you begin.

My internet is unreliable here. Is that a problem?

Often manageable. Video needs a stable connection more than a fast one, and a phone on cellular data sometimes works better than rural home service. If it keeps interrupting sessions, tell your therapist and they will work through it with you.
