Cortland sits between Syracuse and Binghamton, close to both and served properly by neither. It is a county seat and a college town of about 17,500, and the local caseloads reflect a demand much larger than the city. We work with adults here by secure video.

The Crown City, between two others

Cortland is the seat of Cortland County, in New York’s Southern Tier, sitting almost exactly between Syracuse to the north and Binghamton to the south. It is known locally as the Crown City. The Tioughnioga River runs south past it, and the town of Cortlandville surrounds the city entirely.

Being between two larger cities is a familiar and unhelpful position. Both are close enough that Cortland is never treated as isolated, and far enough that every appointment in either is a genuine trip. Roughly thirty-five miles in each direction, on roads that are unremarkable in September and considerably less so in February.

A county seat and a college town at once

Cortland carries two loads that both exceed its size.

It is the county seat, so the courts, county offices and the professional services attached to them are here, drawing people in from every town in the county. And it is a college town, home to SUNY Cortland, which adds several thousand students to a city of about 17,500 for most of the year.

Both of those inflate demand for outpatient mental health care well past what a city this size can supply. The result is what you would expect: local waitlists that do not reflect the population figure on paper.

Anxiety is the most common presentation, and in this county it usually attaches to something concrete: work, money, health, an aging parent, or the ordinary strain of a place where wages have not kept pace with costs.

Depression is second. Winters here are long and grey, the city has had its own industrial contraction, and much of what we see has been present for years before anyone treats it as a treatable condition.

Trauma is a meaningful share and responds well to structured, evidence-based work.

A note on students, said plainly

Because SUNY Cortland is a large presence here, it is worth being clear rather than vague.

We see adults 18 and older, which covers most undergraduates, and a student can work with us provided they are physically in New York State during sessions. That last point matters more than it sounds: a student who goes home to New Jersey or Pennsylvania for the summer cannot simply continue sessions from there, because a therapist has to be licensed where the client is sitting.

We are also not affiliated with any campus counseling service. If you are already working with one, running a second course of therapy alongside it usually does more harm than good, and we would rather tell you that than take the booking.

The between-two-cities problem, in practice

Worth stating concretely because it is the actual alternative people weigh.

A weekly appointment in Syracuse or Binghamton is roughly seventy miles round trip plus the session. That is close to two and a half hours committed for fifty minutes of therapy. People try it, and for a few weeks it works, and then a snow day or a late meeting breaks the pattern and it does not recover.

The value of video here is not that it is more convenient. It is that the course of therapy actually finishes. Twelve consecutive weeks beats seven scattered ones by a wide margin, and consistency is a large part of what makes structured therapy effective at all.

How care is structured

Four stages, in a fixed order.

Intake first, ending with a 0 to 10 intensity rating that becomes your baseline. Your history second, across history, health, relationships and stressors, with any question declinable. Treatment plan third, written with you in your own words, including one goal you choose.

Then weekly sessions, with a monthly review against standardized measures. Your therapist shows you the trend. If it is flat, the plan changes rather than the expectations on you.

The main modality is CBT: structured, present-focused, skills-based. Your therapist will say when a different approach fits your situation better.

For the outpatient concerns we treat, research comparing video-delivered CBT with in-person CBT has generally found comparable outcomes. We keep that claim narrow because that is what the evidence supports.

  2. The wider picture

    Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.

  3. Building the plan

    You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
