Canandaigua is the Ontario County seat, at the north end of Canandaigua Lake and about twenty-five miles from Rochester. It is close enough to be treated as a Rochester suburb and far enough that using Rochester services is a real commitment. We work with adults here by secure video.

The county seat at the top of the lake

Canandaigua is the seat of Ontario County, sitting at the northern end of Canandaigua Lake, one of the Finger Lakes. It is about twenty-four miles southeast of Rochester, sixty-eight west of Syracuse, and it is often described as Rochester’s southernmost suburb, which is accurate and also slightly misleading.

The city is about 10,600 people and is surrounded entirely by the separate Town of Canandaigua. Some county offices sit in the neighbouring town of Hopewell. Finger Lakes Community College has its main campus just east of the city.

Suburb or small city, depending on the question

Whether Canandaigua counts as part of Rochester is a genuine ambiguity here, and it produces a specific and frustrating access pattern.

For commerce and commuting, it functions as the far edge of the Rochester metro. Plenty of people drive up the 332 for work, shopping and specialist medical care.

For services, it functions as a small city on its own. The Rochester practices that dominate the region’s provider directories are twenty-five miles up the road, and twenty-five miles each way is an hour of driving added to every appointment before you count parking.

The result is that Canandaigua residents are counted inside a well-served metropolitan area on paper while facing a small-city reality in practice. Nobody classifies this as an underserved market, and the appointment is still an hour of driving away.

The commuter’s hour, and why it decides this

Worth being concrete, because it is the reason people here choose video rather than a Rochester practice.

A 5pm appointment in Rochester means leaving Canandaigua around 4pm and getting home near 7pm. That is three hours removed from a weekday evening for fifty minutes of therapy, and it has to happen every week for months.

People do it for a while. Then a meeting overruns, or the weather turns, and the appointment slips. Once it slips twice it usually stops.

CBT is the main approach: structured, present-focused and skills-based, which also makes progress visible rather than a matter of impression.

Video, stated carefully

For the outpatient presentations we treat, research putting video-delivered CBT alongside the in-person form has generally found comparable outcomes. We will not oversell that.

Video also has limits. If your needs sit beyond weekly outpatient care, we say so and help you find the appropriate level instead of retaining you on the schedule.

The first three sessions follow a clear structure, so you always know what is coming next.

  1. The first meeting

    Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.

  2. The wider picture

    Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.

  3. Building the plan

    You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
