Glens Falls is a city of about 14,800 in Warren County, at the southern edge of the Adirondack Park, where the Hudson River drops over the falls the city is named for. It is the central city of its own metropolitan statistical area, which is unusual for a place this size and tells you something important about its role.

It functions as the hospital, retail, legal and professional center for Warren and Washington counties and for a swathe of the southern Adirondacks beyond them. Glens Falls Hospital serves a region far larger than the city.

A regional center of 14,800

That role is the source of the access problem here, and it is a sharper version of the county-seat pattern.

A seasonal economy, and a year that is not flat

This is a real feature of life here rather than background colour, and it shapes both what people bring in and when they can be seen.

The regional economy leans heavily on tourism: Lake George in summer, the park year-round, and the businesses built around both. That produces a compressed season of very long hours followed by a much quieter stretch, and for a lot of households an income that is genuinely uneven across the year.

Two clinical consequences worth naming. Anxiety attached to unpredictable income is a distinct presentation from anxiety attached to a demanding salaried job, and treating it usefully means engaging with the actual financial pattern rather than offering generic reassurance. And the off-season is when the mood problems surface, once the distraction of sixteen-hour days is gone and a dark, quiet November arrives.

Scheduling has to bend to that. Sessions at the edges of the day during the season, more availability in the off-season, and a treatment plan that anticipates the shape of the year rather than assuming a flat one.

What people come in for here

Anxiety is the most common, in the form above and in the ordinary version everyone recognizes.

Depression is second, and the seasonal pattern here is pronounced. Long dark winters at this latitude affect sleep, activity and mood in ways that are well recognized.

Grief is a significant share, frequently connected to caring for aging parents in a region where many adults are doing exactly that.

Trauma comes up regularly and responds to structured, evidence-based treatment.

Couples work is common, particularly in households built around seasonal or self-employed income.

Winter is a continuity problem

The stretch from November through March at the edge of the Adirondacks is long, dark and snowy, and rural roads in Washington County are not cleared on a city schedule.

It is also exactly when a course of therapy is most likely to break, because that is when the appointment requiring a drive becomes the appointment that gets rescheduled and then dropped. A therapy course with a six-week hole in February is a materially different thing from one without.

A session that starts at your kitchen table does not get cancelled for weather. In this region that continuity is the strongest practical argument for how we work.

How care is structured

Four stages, fixed order, and worth planning around a year that is not evenly loaded.

Intake comes first, closing with a 0 to 10 intensity rating that becomes your baseline. Your history is second, taking in history, health, relationships and stressors, with any question declinable. Treatment plan is third, written with you in your own words, including a goal you choose.

This is the stage to raise a seasonal work pattern. A plan built for a flat year fails in July here, or in January.

Then weekly sessions and a monthly review against standardized measures. Flat trend, changed plan.

CBT is the primary approach: structured, present-focused, skills-based.

Comparisons between video-delivered CBT and in-person CBT have generally found similar outcomes for the outpatient concerns we treat. Stated exactly that narrowly, because overselling it would be dishonest.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.

My work is seasonal. Can therapy still work on that schedule?

Usually yes, and it is worth planning for rather than working around. Tell your therapist your seasonal pattern at the treatment-plan stage and the schedule and goals can be built to fit a year that is not evenly distributed.

Does winter weather affect sessions?

That is one of the clearer arguments for video here. Weather does not cancel a session that never required a drive, and continuity through a Adirondack-edge winter is often exactly where telehealth outperforms an in-office arrangement.
