Little Falls is a small Herkimer County city in the Mohawk Valley, and at about 4,600 people it is the second smallest city in New York State. A place that size has no therapy market of its own. We see adults here by secure video.
The second smallest city in the state
Little Falls had a population of 4,605 at the 2020 census, which makes it the second smallest city in New York State, ahead only of Sherrill.
It sits in a narrow, rocky gorge where the Mohawk River drops about forty-five feet in less than a mile, which is the feature the city is named for and the reason it exists. That drop blocked river travel, and the locks built to get around it, first the Western Inland Lock Navigation Company and later the Erie Canal, made this a place where goods had to stop, transfer and be handled.
The canal made it. The decline of canal and rail traffic unmade a good deal of it.
No local market, and that is the point

Anxiety is close behind, generally attached to concrete circumstances: employment, money, health, an aging parent, distance from services.
Grief is a substantial share, frequently layered, in a valley where families stay in the same towns across generations.

Isolation in a narrow valley
A clinical point specific to the geography here rather than a general one.
Little Falls sits in a gorge, with steep sides and a strung-out valley of small towns on either end. In winter that means short daylight, real snow and drives that are genuinely unpleasant rather than merely inconvenient.
Isolation in a place like this is physical, not just social. It can mean not seeing anyone outside your household for several days because there is nowhere within reach that is open and the road is bad. That is a different problem from feeling disconnected in a crowded place, and it needs a different plan.
Behavioral activation, the core CBT approach to depression, has to be built from what is actually available in this valley in February. A generic plan assuming gyms, cafés and a social calendar fails here in the first week. Your therapist will build one that fits where you actually live.
How care is structured
Four stages, in a fixed order.
Intake first, closing with a 0 to 10 rating of intensity that becomes your baseline. Your history second, covering history, health, relationships and stressors, with every question declinable. Treatment plan third, written with you in your own language, including one goal you choose.
Then weekly sessions, with a monthly review against standardized measures. Your therapist shows you the numbers. A flat trend means the plan is revised.
The primary approach is CBT: structured, present-focused, skills-based. In a valley where a week can pass without much external input, the skills practice between sessions is doing real work rather than filling time.
Video, stated plainly
Studies comparing the two formats have generally found video-delivered CBT comparable to in-person CBT for the outpatient concerns within our scope. We are careful with that sentence because it is easy to overstate.
