Yes, and telehealth is permanent for mental health

The point worth leading with, because it causes real confusion: mental health services delivered by telehealth are a permanent Medicare benefit. Coverage rules for many other Medicare telehealth services have been extended, revised and allowed to lapse repeatedly over the last several years, and a lot of people reasonably assume mental health sits in that same uncertain category.
It does not. You are not relying on a temporary allowance that might disappear mid-treatment.
What you pay depends on which Medicare you have
This is where most of the confusion actually lives, and “Medicare” is really three different situations.
Original Medicare on its own. Part B covers outpatient mental health, and you are generally responsible for 20 percent coinsurance after your annual Part B deductible. There is no cap on that 20 percent, which is why most people carry something alongside it.
Original Medicare plus a Medigap supplement. The supplement typically absorbs some or all of that 20 percent. For many members this is the difference between owing a meaningful amount per session and owing very little.
Medicare Advantage. An Advantage plan replaces Original Medicare’s cost structure entirely with its own copays, and adds its own network rules, some of which require referrals. It is not a variation on Original Medicare so much as a different product.
We determine which of the three describes you, and what it means in dollars, before your first session rather than after a statement arrives.
The couples counseling rule
Worth stating plainly because it disappoints people who find out late. Medicare covers treatment of a diagnosed condition.
That means relationship counseling, where the relationship itself is the reason for the sessions, is generally not a covered Medicare benefit. Family or joint sessions held as part of treating your diagnosed condition can be covered.
The distinction is real and we will tell you which situation applies to you before you are billed for anything.
Why this matters in Long Beach specifically
Long Beach has a substantial year-round older population, considerably more than a barrier-island resort reputation suggests. That makes Medicare one of the most common ways therapy gets paid for in this city.
It is also a city where getting to an appointment is a genuine obstacle for exactly this population. Every route off the island crosses water. Winter weather, night driving and the summer traffic that clogs the bridges all fall hardest on the people most likely to be on Medicare.
A session that requires no trip removes that entirely.
The out-of-state caveat, which comes up a lot here
Our clinicians are licensed in New York. Sessions can continue anywhere in New York State but cannot continue while you are physically in another state, because licensure follows where you are sitting.

Care opens with an intake and a 0 to 10 baseline rating. The early work maps your history, and the treatment plan is written with you. Then weekly sessions and a monthly review using standardized measures.
For the outpatient concerns we treat, research comparing video-delivered CBT with in-person CBT has generally found comparable outcomes. If your needs exceed weekly outpatient care, we will say so and help you find the right level.

Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.
- Mapping what is behind it
Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.
- Writing the plan together
You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.
- Ongoing
Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.
Therapy here is measured, not guessed
Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
Sessions are weekly for the first two months to build a foundation, then frequency is reassessed with you. You set the pace, and you share only what you are comfortable sharing.

Does Medicare cover therapy by video?
Yes. Mental health services delivered by telehealth are a permanent Medicare benefit, not a temporary pandemic allowance. That distinction matters because the rules for other telehealth services have changed repeatedly while mental health coverage has been made permanent.
What do I actually pay under Original Medicare?
