The actual numbers for 2026
Under Part B, outpatient therapy is covered. The 2026 Part B deductible is $283 (CMS, 2026 fact sheet). Once it is met, Medicare pays 80 percent of the approved amount for a session and you pay 20 percent.
Two layers commonly change that. A Medigap supplement usually absorbs the 20 percent, which can make your therapy sessions cost nothing out of pocket. A Medicare Advantage plan swaps the whole 80/20 structure for its own copay per session. Which situation is yours determines your cost, so telling us what is in your wallet is step one, and quoting your real number is step two. We do both before you book a first session.
A city that retired well
Kodak and Xerox built Rochester’s twentieth century, and the workforce that built those companies is now the city’s large retired generation. That makes Medicare, more than any commercial carrier, the coverage story that matters here. It also means we regularly see people starting therapy at 68 or 75, some for grief, some for the strange quiet after a working life ends, some for anxiety that finally has room to be noticed. Age has never once been a reason therapy stopped working.
Turning 65 without losing your therapist
The Medicare transition trips people mid-treatment: employer coverage ends, the new card arrives, and they assume the therapy has to end or restart somewhere new. It does not. Outpatient mental health continues under Part B. If you are approaching 65 and in therapy, or thinking of finally starting once the working years wind down, the move is administrative, not clinical: new card, new verification, same weekly session. We have walked plenty of clients through it, and the checking is our job rather than yours.
What Medicare will not pay for
Worth stating in advance: relationship counseling for its own sake is generally outside Medicare’s coverage, because Medicare pays for treatment of a diagnosed condition. Family involvement in your own treatment can be covered. We determine which side of that line your situation falls on before any bill exists.
What therapy with us involves
The work opens with an intake and a 0 to 10 baseline rating. What follows you can edit freely by declining questions, and the treatment plan is co-written. Sessions run weekly after that, with standardized measures reviewed monthly so the trajectory is visible. Where progress stalls, the plan is rewritten.
On format: video-delivered therapy has performed on par with office-based care in comparative research on outpatient concerns such as depression, anxiety, and grief. And to be equally plain about geography, we hold no Rochester office; Buffalo, 75 miles off, is the nearest, which is why Rochester care is video care.

- Writing the plan together
You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.
- Ongoing
Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.
Therapy here is measured, not guessed
Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
