Staten Island keeps Original Medicare at a higher rate than New York State as a whole, in keeping with a borough that mistrusts anything that changes a working arrangement. Of the island’s nearly one hundred thousand Medicare-eligible residents, just under half have moved to private Advantage cards, a smaller share than the statewide 53 percent.

Part B, your supplement, and the real number

The Part B arithmetic runs: deductible first, $283 in 2026 by Medicare’s setting, then 80/20, Medicare paying 80 percent of the approved amount while your coinsurance is 20. The wildcard that decides your actual cost is the Medigap supplement, which many islanders carry and which often covers the 20 entirely.

No referral is needed, the annual depression screening is free, and one boundary is worth stating early: counseling aimed purely at a relationship is not a Part B benefit, though family involvement in treating your own condition is.

What Medicare covers here

  • Medicare Part B covers outpatient mental health care. The 2026 deductible, set by Medicare, is $283; after it, Medicare pays 80 percent of the approved amount per session and your share is 20 percent.
  • A Medigap supplement frequently covers that 20 percent in part or in full. Bring the supplement card to verification and we fold it into your confirmed number.
  • Mental health telehealth from home is a permanent Medicare benefit, with the periodic in-person requirement waived through December 31, 2027. No referral needed, and the annual depression screening is free.
  • Original Medicare covers family involvement only as part of treating your own condition, not counseling aimed solely at a relationship, stated here so nothing surprises you later.

What does therapy here actually look like?

The first three sessions follow a clear structure, so you always know what is coming next.

  1. Getting started

    Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.

  2. Mapping what is behind it

    Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.

  3. Writing the plan together

    You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.

  4. Ongoing

    Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.

Therapy here is measured, not guessed

Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
