Why MindView
Most therapy in Tonawanda has no structure. Ours has four stages and a scoreboard.
Plenty of practices take Medicare. What you get here is one format for every client: a full intake, a written treatment plan, and standardized measures reviewed with you every month, so you can see whether it is working.
- 01
Intake
What is actually going on, rated 0 to 10 as your baseline.
- 02
Your history
Why it is happening. History, relationships, work, health.
- 03
Treatment plan
Written and collaborative, built against those causes.
- 04
Monthly review
Standardized measures, reviewed with you. You see the trend.
A license is where our training starts, not where it ends.
Every clinician is state-licensed before they see a single client. Then they train with us across every modality, and how to match one to the problem in front of them.

Medicare in Tonawanda, in short
Yes. MindView Therapy is enrolled with Medicare and sees City of Tonawanda adults by secure video anywhere in New York State. Medicare places no geographic restriction on mental health telehealth and the home is a permitted site. Part B covers outpatient therapy after the annual deductible and then 20 percent coinsurance, and whether anything sits underneath to absorb that 20 percent is the question we check before booking.
Of the 3,101 City of Tonawanda residents aged 65 and over, 2,977 hold Medicare and 439 also hold Medicaid or other means-tested coverage, 14.2 percent. Statewide the figure is 21.4 percent. For most people over 65 in this city there is no second public layer underneath the Medicare card, which means the 20 percent coinsurance is a number somebody actually pays rather than one that quietly disappears.
What is not underneath your Medicare card
Of the 3,101 City of Tonawanda residents aged 65 and over, 2,977 hold Medicare coverage, 96.0 percent. That share is roughly what you would expect for an age group where Medicare is the default. What is more useful for planning a therapy budget is what else those 2,977 people carry alongside it.
Only 439 of them, 14.2 percent, also hold Medicaid or another form of means-tested public coverage. Erie County sits close to the same mark, 26,055 of 178,628 residents 65 and over, 14.6 percent. Statewide the figure is 739,296 of 3,462,393, 21.4 percent, close to a third higher than the City of Tonawanda’s rate. That gap matters, because for most people over 65 in this city, Medicare is standing alone rather than working alongside a second public program that would absorb part of what it does not pay.
A coverage rate is not a statement about health, income, or need, and the 439 figure carries a margin of error of plus or minus 178. What it does say, directionally, is that the arithmetic in the next section applies to more people here than it would somewhere with a larger second layer underneath, and that it is worth writing out in full rather than waving at in passing.
The twenty percent, and who pays it
Medicare Part B covers outpatient mental health care. In 2026, the annual Part B deductible is $283, meaning that amount is owed before Part B starts paying its share of covered services for the year. Once the deductible is met, Part B pays 80 percent of the allowed amount for a covered visit, and the remaining 20 percent is coinsurance, paid by whoever holds the card unless something else picks it up.
The standard 2026 Part B monthly premium is $202.90, and it is a separate cost from the deductible and coinsurance, charged whether or not any care is used that month. None of these three figures substitute for each other, and a page that only mentions the premium, or only the deductible, has not actually answered what a session costs.
That 20 percent coinsurance is exactly the piece a Medicaid benefit or another means-tested program typically absorbs when a person has one. In a city where most people over 65 do not, the coinsurance is not a rounding error on a statement. It is a real weekly number, and it is the one we work out with you before your first session rather than after it arrives on a bill.
Outpatient therapy under Part B does not require a referral from a physician. You can book directly, and we confirm the deductible status and the coinsurance figure that applies to your specific situation as part of setting up your first appointment, so the number is known before the session rather than discovered afterward.
If your card says Advantage
A Medicare Advantage card is a different conversation from original Medicare, and we do not assume network status for an Advantage plan the way the arithmetic above assumes it for Part B. Advantage plans are sold by private insurers, and each one sets its own network, referral rules, and cost sharing, so two Advantage cards can work in two different ways even when both say Medicare on them.
Some Advantage plans use a flat copay per visit instead of the 20 percent coinsurance structure above, and some carry a different deductible or none at all. Reading the plan name off a card does not tell us which of those applies, which is exactly why we do not guess.
In practice that means we check which card you carry before anything gets booked, original Medicare or a specific Advantage plan, and confirm what applies rather than quoting a Part B number and calling it close enough. We do not advise on which supplemental coverage to carry or recommend switching plans; that decision sits with you and whoever helps you compare plans each year. Our job is telling you what your specific card means for a session here, not steering the choice of card itself.
Seen at home, by rule
Medicare places no geographic restriction on mental health telehealth, and the home is a permitted site under the federal rule. That is different from how Medicare treats some other kinds of telehealth visits, and it is the reason a City of Tonawanda resident does not need to travel anywhere to use this benefit.
Every session is online, by secure video, with a clinician licensed in New York. MindView has no office in the City of Tonawanda, and none is implied anywhere here. The video visit is not a lesser stand-in for an in-person option being withheld; it is the only way sessions happen at any of our locations, for every client, regardless of age or plan.
Because the requirement is licensure in New York rather than proximity to a particular office, a City of Tonawanda client is not limited to whoever happens to be closest. Any clinician licensed in the state is available, and you only need to be physically in New York for the session itself, whether that is a home in the City of Tonawanda or somewhere else in the state on a given week.
There is no address, no set of office hours, and no map listing here, and that is deliberate rather than an oversight. A telehealth-only benefit does not carry the same local footprint an in-person clinic would, and nothing above should read as if it does.
What the years after 65 bring to a first session
Among City of Tonawanda residents aged 65 and over, the 14.2 percent who hold a second layer of coverage are the minority. None of that describes why any particular person books a first session, but it describes a stage of life where the reasons tend to repeat: grief, a diagnosis, and a social circle that quietly gets smaller.
Grief, a social circle that has quietly gotten smaller, and the ordinary strain of retirement or a new health diagnosis are common things a person carries into a first session without ever stating them as the reason for calling. That is normal after 65, not evidence that something has gone wrong, and an intake makes room for it without requiring a diagnosis first.
The financial side is not separate from that. When the honest answer to what a session costs is a specific weekly number rather than a shrug, the more useful move is putting that number in front of someone before they decide, so the decision left is whether they want the hour, not what it might turn out to cost.
None of this is a claim about what any individual booking here is dealing with, and it is not a reason to wait for things to feel worse before calling. It holds whether the reason is grief, ordinary depression or anxiety that reads differently after 65 than it did decades earlier, or a hard transition tied to health or retirement.
A first session itself is an intake rather than treatment: what brought someone in, the background that matters to it, and what they would want different, with progress tracked afterward using standardized measures rather than a protocol name attached to the visit. Nobody needs a diagnosis in hand to book that conversation.
People come in for grief, for depression and anxiety tied to a change in health, for life transitions that come with retirement or a move, and for loneliness that a smaller circle makes harder to talk through alone. An adult child helping a parent look into this is welcome to start the same way anyone else would, through the booking page, where the coinsurance figure gets confirmed before the first session is set.
What Medicare covers here
- •Part B covers outpatient mental health care. The 2026 annual Part B deductible is $283, then 20 percent coinsurance applies. The standard 2026 Part B monthly premium is $202.90.
- •In the City of Tonawanda, most people over 65 do not hold a second means-tested coverage layer, so that 20 percent coinsurance is typically paid directly rather than absorbed elsewhere.
- •Medicare Advantage plans are not treated as automatically in-network. We verify which card you carry before booking.
- •No referral is required for outpatient therapy under Part B.
- •Sessions are held from home by secure video, a permitted site under the federal telehealth rule for mental health care.

Your first session is an intake, by secure video from home, with a clinician licensed in New York.
What does therapy here actually look like?
The first three sessions follow a clear structure, so you always know what is coming next.
- Getting started
Your first session is an intake. Your therapist asks what brought you in and about your history, and you rate the intensity of what you are feeling from 0 to 10. That becomes your baseline. You set a recurring weekly time before you leave.
- Mapping what is behind it
Your therapist walks through your life across stages, looking for the patterns and strengths behind what brought you in. You can decline any question and keep any answer short.
- Writing the plan together
You and your therapist build the plan together. Goals are tied to what you came in for, each with concrete objectives, plus one personal goal that matters to you.
- Ongoing
Weekly sessions work the plan. Once a month you and your therapist review standardized measures together to see whether it is working, and the plan is adjusted from what they show.
Therapy here is measured, not guessed
Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
Sessions are weekly for the first two months to build a foundation, then frequency is reassessed with you. You set the pace, and you share only what you are comfortable sharing.

Medicare questions
What will Medicare actually leave me to pay for a therapy session in the City of Tonawanda?
The 2026 Part B annual deductible is $283. After that is met, Part B pays 80 percent of the allowed amount and the remaining 20 percent is your coinsurance. The standard 2026 Part B premium is $202.90 a month. We take those figures and your specific deductible status and tell you the number that applies to your first session before you book.
My card says Medicare Advantage. Is that the same as regular Medicare?
Not automatically. Advantage plans are sold by private insurers and each one sets its own network and cost sharing, so we do not assume network status the way we can with original Medicare. We check which specific card you carry, original Medicare or an Advantage plan, before anything is scheduled, and confirm what it means for your session.
Do I need a referral from my doctor before starting therapy?
No. Outpatient therapy under Medicare Part B is self-referred, so you can book directly without seeing another provider first. We still confirm your deductible status and coinsurance before your first session, but that step does not require a referral from anyone.
Can I actually be seen from home in the City of Tonawanda, or do I need to travel somewhere?
You can be seen from home. Medicare places no geographic restriction on mental health telehealth, and the home is a permitted site under the federal rule. Every session is by secure video with a clinician licensed in New York, and MindView has no office in the City of Tonawanda for you to travel to.
Why does the Medicaid coverage rate matter if MindView does not take Medicaid?
MindView holds no Medicaid contract, and it is not described as an option here. The 439-person figure for City of Tonawanda residents over 65 is reported only to show how many people have a second coverage layer sitting under Medicare. For everyone else, the 20 percent coinsurance under Part B is the number that applies, and that is what gets verified before booking.
