Madison, Indiana is the Jefferson County seat on the Ohio River, 48 miles northeast of Louisville and 68 miles southwest of Cincinnati. It was the third-largest city in Indiana by 1850 on river commerce, then the railroads went elsewhere and the growth stopped - which is exactly why its 19th-century downtown survived. A city preserved by being passed over

Madison, Indiana was, by 1850, the third-largest city in the state. The Ohio River made it. Commerce came off the water, the town grew fast, and it built accordingly - mansions, a courthouse, a dense commercial downtown built in brick and stone by people who assumed the growth would keep going.

Then the railroads routed around it, and it stopped. What follows in most places is demolition and replacement over the next century. What happened here instead is that nothing got torn down, because there was no money and no pressure to tear anything down. The result is the Madison Historic Landmark District, covering 133 downtown blocks, the Lanier Mansion, the Jefferson County Courthouse, and a riverfront town that looks close to what it looked like when it was the third-largest city in Indiana. Clifty Falls State Park borders the city.

That history is now the economy. Preservation and tourism carry a real share of the local working life, which means seasonal work, seasonal income, and a town that is very busy for part of the year and very quiet the rest of it.

Geographically Madison faces away from Indiana. Louisville is 48 miles southwest, Cincinnati 68 miles northeast, and Indianapolis is not the city people here drive to. Once a month you complete standardized measures, and the two of you read the trend. No movement means a different approach. If the numbers are not moving, the plan changes rather than drifting. That is the entire reason to measure anything.

The therapy is shaped around your problem and grounded in cognitive behavioral therapy, which is structured and skills-driven. You leave with something to practice. Your therapist is licensed in Indiana, which sets one rule: sessions happen while you are physically in Indiana. Your house in Madison, Indiana qualifies; a hotel room across the river does not.
