What DBT actually is
Dialectical behavior therapy is a structured, skills-based therapy. The word “dialectical” points at the tension it is built around: you are doing the best you can with what you have, and some of what you are doing is not working. Both are true at once. Therapies that only push acceptance leave you stuck; therapies that only push change tend to feel like being told the problem is you. DBT holds the two together deliberately.
The content is four sets of skills. Mindfulness is paying attention to what is happening without immediately reacting to it. Distress tolerance is what you do in the worst ten minutes, when the feeling is at its peak and every option looks like a bad one. Emotion regulation is the slower work of changing how often those peaks happen. Interpersonal effectiveness is asking for something, or saying no, without the conversation detonating.
Being straight about what we offer and what we do not
This matters more in DBT than in most modalities, and a page that skips it is not being useful.
Full-model DBT is a program, not a therapy hour. It has four parts: individual therapy, a weekly skills group, phone coaching between sessions, and a consultation team for the clinicians. Research on DBT largely studied that whole package.
We provide DBT-informed individual therapy. We do not run a skills group, and we do not offer between-session phone coaching. For a lot of people that is the right amount of structure. If you need the full program, that is a real and different thing, and we would rather tell you now than four sessions in. What a session involves
A session has a shape. You bring the week, and if something went badly you take it apart in detail, moment by moment, tracing what happened before the reaction and what followed it. That sounds tedious and it is the most useful thing in the method, because the chain is where the intervention points hide. Somewhere in that sequence is a link where a skill would have fit.
Then you learn or sharpen that skill and agree how you will practice it. Practice is not decoration. Skills you have only discussed are not available at 2am; skills you have rehearsed are.
What DBT is used for
DBT was built for people with intense, fast-moving emotions and impulsive reactions, and that is still where the evidence is strongest. It is now used more broadly for anger, mood swings, relationship struggles, and stress management, and it is often useful alongside anxiety and depression work when the difficulty is less about the content of the thoughts and more about the intensity underneath them.
It is not the right first answer for everything. If the problem is a specific fear you have organized your life around avoiding, a targeted exposure protocol usually gets there faster. Questions people ask before booking
Do I need a borderline personality disorder diagnosis to do DBT? No. DBT was developed with that population and the skills were never restricted to it. You do not need any particular diagnosis to ask about it.
Is this the full DBT program with a skills group? No. We provide DBT-informed individual therapy. There is no skills group and no phone coaching here. If you need the full program we will say so at intake.
I live in Queens but work in Manhattan. Does that matter? Only in one practical way: you need to be physically in New York State during the session, which both boroughs satisfy. Where you log in from is your choice, as long as it is private enough to talk.
Will I get worksheets? Some, and they are brief. The point of a DBT worksheet is to make a skill usable in the moment, not to give you homework in the school sense.
How long does it take? Skills work usually runs several months. That is a range rather than a promise, and it depends on what you are working on.
