What DBT actually is
Start with the odd word. “Dialectical” names a tension the therapy refuses to resolve: you are doing the best you can, and some of what you are doing is not working. Both are held at once, on purpose. Approaches that only validate leave you where you are, and approaches that only push change land as an accusation. DBT does both in the same hour, which is why it suits people who found other therapy either too soft or too blaming.
What you actually learn is four sets of skills. Mindfulness, which is noticing what is happening before reacting to it. Distress tolerance, for the peak, when the goal is getting through without making it worse. Emotion regulation, the slower work of making the peaks less frequent. Interpersonal effectiveness, for asking or refusing without the conversation going up in flames.
That last one does specific work in Buffalo, where a lot of people live within a few minutes of the family they grew up with and cannot simply go quiet for six months after a bad exchange.
What we offer, and what we do not
Full-model DBT is a program rather than a therapy hour: individual sessions, a weekly skills group, phone coaching between sessions, and a consultation team behind the clinicians. Most of the research studied that whole package.
What we provide is DBT-informed individual therapy. No skills group, no between-session phone coaching. For many people that is the right amount of structure, and for some it is not enough. That is a real distinction and you should have it before you book, not after. Ask at intake and you will get a direct answer about which one fits your situation.
If you need the full program, the honest position is that comprehensive DBT is thin on the ground across Western New York, and the wait for it can be long. That is worth knowing before you decide, and we would rather you hear it here than discover it after four sessions with us.
What a session involves
You bring the week. When something went badly, you walk it back link by link: what happened, what you noticed, what you did, what followed. This is called a chain, and going through it slowly is the most useful thing in the method, because somewhere along that sequence there is a point where a different move was available. That point is the target.
Then you learn or sharpen the skill that fits there, and you agree how to rehearse it. Rehearsal is the whole point. A skill you have only talked about is not available when you need it. A skill you have practiced is.

What DBT is used for
The method was developed for people whose emotions arrive fast and hit hard, and that remains where the evidence is strongest. It is now applied more broadly to anger, mood swings, relationship struggles and stress management, and it often runs alongside work on anxiety or depression when the obstacle is intensity rather than content.

The first advantage is that distress tolerance skills are, by definition, for the moment the distress happens. Questions people ask before booking
Do I need a specific diagnosis for DBT? No. It was developed with one population and the skills were never limited to it. You do not need a diagnosis to ask whether it fits.
Is this the full program with a skills group? No. Individual DBT-informed therapy only, with no group and no phone coaching. If you need the full model we will tell you at intake.
I am in Buffalo but travel for work. Does that break things? Only if you leave the state. You need to be physically in New York during a session because that is where your therapist is licensed. Within New York, where you log in from is up to you.
Does DBT work over video? The skills are behavioral and portable, which is what makes them transfer. Rehearsing one in the environment where you need it is arguably easier by video, not harder.
How long does it take? Skills work usually runs several months, depending on what you are working on.
