OCD collects its toll at the two ends of the day: the checking that makes you late leaving, and the rituals waiting when you get back, and in a commuter city those are precisely the hours nobody can spare.

What ERP actually is

Before it costs anything else, OCD costs minutes. The stove, then the locks, then the taps, then the photo of the stove taken as proof, and the sequence still has to run once more because the feeling of certainty never arrived. Rituals persist because they buy a moment of relief, and that moment convinces the brain they were necessary. The loop tightens every time it pays out.

Exposure and response prevention unwinds it from the behavior side. You approach the situations that set the alarm off, deliberately and in planned steps, and you practice not running the sequence afterward. Denied the ritual, the anxiety peaks and then subsides by itself, and with repetition the alarm quiets. ERP is the most studied form of CBT for OCD and a first-line treatment, which describes the evidence base rather than promising your result.

What a session involves

The plan comes before any exposure. You and your therapist rank your triggers into a hierarchy, easiest at the bottom, and name the response-prevention targets, the specific checks and reassurances you will practice leaving undone. Work proceeds one agreed step at a time; the top of the list waits until the bottom has gone quiet.

Progress gets measured the same way the problem does: concretely. Between-session practice carries most of the load, and once a month you review standardized measures with your therapist. If a fair trial is not moving the numbers, the plan is rebuilt.

What ERP treats

ERP is the dedicated protocol for OCD, including checking, contamination fears, mental rituals and reassurance-seeking. The same graded structure treats phobias, panic, agoraphobia, social anxiety and avoidance-driven anxiety. No diagnosis is required to ask about fit. Worth knowing: many of the costliest rituals are invisible, the mental review on the train, the question asked twice in different words. Those count, and response prevention targets them the same way it targets a lock check.

ERP in Yonkers

In Yonkers, 23.1% of workers commute by public transportation (US Census Bureau, ACS 2020-2024 five-year estimates, table B08006), and a fixed train or bus time is unforgiving arithmetic for a morning ritual. Questions people ask before booking

I catch an early train and get home late. When would sessions even happen? In the evening, by video, with no further trip attached, and some schedules fit an early or midday slot instead. This gets solved at booking, not discovered after.

Can ERP work on the rituals that make me late in the morning? Yes, directly. The hierarchy can be built around your leaving-the-house sequence, and the practice happens on real mornings where the problem actually runs.

Will I be pushed too fast? No. Steps are ranked, agreed, and taken from the easy end first. The pace is set with your therapist, and consent runs through the whole protocol.

Is ERP only for OCD? No. The same exposure structure treats phobias, panic and social anxiety, with the hierarchy built around the feared situation.

Does video ERP actually work? ERP has been delivered and studied by video. Your exposures happen in your real environment, which is where the problem lives anyway.
