When the house is also the office, OCD gets an all-day venue: the sink is next to the desk, the lock is the boundary of the workplace, and the ritual no longer has to wait for five o’clock.

What ERP actually is

The loop that runs OCD does not care about the venue. A trigger raises fear or doubt, a ritual buys relief, and the relief certifies the ritual, so the loop returns with interest. What the venue changes is opportunity, and a home that doubles as a workplace offers the loop more openings per day than any commute ever did.

Exposure and response prevention works the loop directly. You approach a trigger in a planned, ranked, agreed step, decline the ritual, and let the anxiety come down on its own, which it does with repetition. ERP is the most studied form of CBT for OCD and a first-line treatment. That describes the evidence base; your own course answers to your own measures.

What a session involves

Structure comes first and consent runs throughout: an exposure hierarchy ranked from easiest to hardest, response-prevention targets naming the rituals you will practice not doing, both built with your therapist at the treatment-plan stage. Sessions pick the next step and rehearse it; the week runs it, several times, in the environment where it lives.

Nothing is improvised on you and nothing starts at the top. Monthly standardized measures tell you both whether the steps are compounding, and a stalled plan gets rebuilt rather than repeated. The between-session repetitions carry most of the change, which is why they are designed to fit inside an ordinary day.

What ERP treats

ERP is the dedicated protocol for OCD, including checking, contamination, symmetry, intrusive thoughts, mental rituals and reassurance-seeking. The same graded structure treats phobias, panic, agoraphobia, social anxiety and avoidance-driven anxiety. No diagnosis is required to ask about fit. The quiet forms count fully: the mental re-run of a sent message, the reassurance sought in careful casual phrasing, the task redone past any standard a colleague would recognize.
