What ERP actually is
The engine of OCD is simple and stubborn. A trigger produces a spike of fear or doubt, a ritual or an avoidance relieves it, and the relief is what welds the loop shut, because it teaches the brain the ritual was what kept you safe. Exposure and response prevention is the structured practice of opening that loop: approaching the trigger on purpose, skipping the ritual, and letting the anxiety fall on its own, which it does, and which the brain slowly relearns.
ERP is the most studied form of CBT for OCD and a first-line treatment. That sentence describes decades of research, not a personal guarantee, and your own course is tracked with measures rather than assumed to match it.
What a session involves
You and your therapist rank your triggers into a hierarchy at the treatment-plan stage, from mildly annoying to currently unthinkable, and define the response-prevention targets, the checks and reassurances you will practice leaving undone. Sessions choose the next step, rehearse it, and review what last week’s practice showed. The steps run in your real week, between sessions, where the triggers actually live.
Nobody is thrown in unprepared. The pace is agreed, the order is yours, and once a month standardized measures show whether the work is compounding. A plan that is not moving after a fair trial gets rebuilt.

Questions people ask before booking
I move apartments almost every year. Does treatment start over each time? No. The method and the skills travel with you. Triggers get re-ranked in the new place, and the course continues rather than restarting.
I leave New York for the summer. What happens to sessions? They pause while you are out of state, because licensure is state-bound, and resume when you are back. Plan the timing at intake so nothing surprises you.
Is ERP safe to do without an in-person therapist nearby? ERP has been delivered and studied by video. Pacing is graded and agreed step by step, and crisis resources are covered at intake before any exposure work begins.
What if my roommates are around during sessions? Headphones and a closed door usually suffice, and privacy logistics are a normal part of booking, not a problem you need to solve alone.
Do you treat health anxiety and checking too? Yes. Ritual- and avoidance-driven anxiety broadly responds to the same structure, health anxiety and checking included.
