Treatment starts from a specific premise: the problem is not effort, it is that certain internal systems, tracking time, holding a plan in mind, resisting a more interesting distraction, do not run automatically the way they do for most people. Demanding more willpower against a system that is not built to supply it just produces more failure and more shame.
Instead we build outside the head what the head will not reliably do on its own: externalized time cues, capture systems placed where you will actually see them rather than where they are supposed to live, tasks reduced to a first step small enough that starting is nearly automatic, and physical environments arranged so the wanted behavior takes the least effort, not the most. This runs through cognitive behavioral therapy adapted specifically for adult ADHD.
A second, quieter piece of the work addresses the accumulated self-story: years of being called scattered or unreliable leaves a mark, and that mark often produces avoidance that gets mistaken for the original symptom when it is really shame doing the driving.
