Finding a clinician who actually treats OCD, rather than just talks around it, is hard everywhere and harder still on Staten Island, where specialist supply is thinner than anywhere else in the city. What OCD looks like

There is no single face to OCD. The visible rituals get most of the attention, but a large share of people with the condition never perform anything an outsider would notice, and those are usually the cases that go the longest without a diagnosis.

Every version runs on the same two parts. An obsession is the intrusive, unwanted thought, image or urge that shows up carrying real distress. A compulsion is whatever you do, physically or mentally, to bring that distress back down, and doing it is exactly what keeps the loop alive:

  • Rereading and rechecking your own words. Going back over a sent text or email, hunting for a phrase that might have landed wrong.
  • Silent bargaining. A private phrase, count or image used to cancel out a thought before it can “count.”
  • Physical checks that repeat. Locks, stoves, the stove again, even after you already know the answer.
  • Body scanning. Checking your own physical sensations for proof that something is wrong.
  • Steering around a trigger. Avoiding a person, a route or an object because contact sets off the whole sequence.

The content usually runs directly against your actual values, which is exactly why it is so distressing rather than easy to dismiss. Common themes include harm, contamination, relationships, sexuality, morality and religion.

How we treat OCD here

Every episode follows the same mechanics: a thought fires, distress spikes, a compulsion brings it back down, and the relief teaches your brain that the thought was a real danger and the compulsion was what handled it. Repeat that enough times and the loop tightens on its own.

That mechanism is exactly why standard talk therapy so often falls short here, and can even reinforce the problem. Spending a session working out whether the feared thing could really happen, or hearing reassurance that it will not, functions as one more compulsion, just performed out loud with a therapist in the room instead of alone.

Exposure and response prevention interrupts the mechanism directly instead of feeding it. It is a defined, staged protocol, not a conversation: you and your therapist build a ranked list of triggers, then move through it on purpose, approaching a trigger and choosing not to perform the compulsion, and letting the distress crest and drop without the usual escape hatch.

You never start at the hardest item. ERP also holds up well by video, and for contamination fears and checking rituals specifically, doing the work in your own kitchen or at your own front door is a genuine advantage rather than a compromise.

OCD in Staten Island

Staten Island reports 15.3 percent diagnosed depression and 14.6 percent frequent poor mental health days among adults, per the CDC’s PLACES 2025 release, both figures near the citywide middle. OCD itself is not broken out in local surveillance data, and the condition is chronically undercounted anywhere access is limited, since a specialty diagnosis depends on reaching a clinician trained to recognize it in the first place.

Access is where Staten Island genuinely separates from the rest of the city. HRSA has issued exactly one mental health shortage designation for the entire borough, scored 18 of 25, treating its low-income population of roughly 65,000 people as a single unit. Every other borough carries several distinct neighborhood-level designations; Staten Island alone is drawn as one continuous zone, matching a borough with the fewest providers in absolute numbers and the lowest population density of the five. Clinicians trained specifically in ERP are a narrower group again within that already thin pool, which goes a long way toward explaining why people here so often spend years in general talk therapy before finding something that actually interrupts the compulsions.

Common questions

Is there an OCD specialist office anywhere on Staten Island? No. Our nearest office is in Jamaica, Queens. ERP for clients here runs entirely by secure video.

Why can regular talk therapy make OCD worse instead of better? Because reassurance-seeking and analyzing the feared outcome both function as compulsions. Feeding the cycle rather than interrupting it is why so many people report years of therapy with no real change.

All of my compulsions happen inside my head. Does that still count? Yes. Mental review, silent bargaining and covert reassurance-seeking are compulsions even without anything visible, and these are among the presentations most often missed entirely.

Does ERP actually work as well over video? Often better, since most real-world triggers already exist in your own home. Sessions run by telehealth anywhere in New York State no matter where on the island you live.

I tried therapy before and it did not help. What would be different this time? There is a real chance ERP specifically was never offered. General talk therapy for OCD is common and frequently ineffective, which points to a treatment-matching gap, not anything about you.
