High-functioning depression describes people who keep meeting their responsibilities, work, family, appearances, while living with persistent low mood, emptiness, or joylessness underneath. Because they keep going, it is easy to dismiss. It often overlaps with persistent depressive disorder (dysthymia), a long-lasting, lower-grade depression. It is not less serious for being hidden, and it responds well to cognitive behavioral therapy, sometimes combined with medication.

From the outside you are managing, working, showing up, taking care of what needs taking care of. Inside, there is a persistent flatness, an emptiness, a sense that you cannot remember the last time you felt genuinely good. That gap is what people mean by high-functioning depression.
Hidden behind the functioning
High-functioning depression is not an official diagnosis, but it names a real and common experience: persistent low mood carried by someone who keeps functioning. Because you keep delivering, others assume you are fine, and you may have decided the same, telling yourself it is not bad enough to count.
It often overlaps with persistent depressive disorder, or dysthymia, a long-lasting, lower-grade depression that can quietly run for years.
Functioning is not the same as fine
The reason high-functioning depression goes untreated is that it does not look like the picture of depression. But continuing to function is not evidence that you are okay. Living for a long time at low mood, running on empty, and feeling no real pleasure is a reason for support, not a standard to keep meeting.
What helps
Cognitive behavioral therapy helps by rebuilding activity and connection, testing the low-grade depressive thinking that has become background noise, and reconnecting you with what once felt meaningful. Because it often travels with burnout and low self-worth, we frequently work on those together. For some people, medication managed by a prescriber is part of the plan.

What does therapy here actually look like?
The first three sessions follow a clear structure, so you always know what is coming next.
- Getting started
Your therapist asks what brought you in and how the low mood shows up beneath the functioning. You rate the mood, energy, and joylessness on a 0 to 10 scale and set a recurring weekly time before you leave.
- Mapping what is behind it
Your therapist walks through your history, looking at how long the low mood has been present, what keeps it running, and the strengths that carried through. You can decline any question and keep answers short.
- Writing the plan together
You build the plan together. Goals target the low mood, the joylessness, and the running-on-empty, with concrete objectives and a plan for re-engaging with what once felt meaningful. You also set one personal goal that matters to you.
- Ongoing
Weekly sessions work the plan. You rebuild activity and connection in planned steps and test depressive thinking. Once a month your therapist reviews standardized measures and adjusts the plan from what they show.
Therapy here is measured, not guessed
Once a month you have a measurement-based care review. You complete standardized measures, and your therapist reviews the trend with you. If something is not working, the plan changes. Regular therapy is the work. The review is the navigation system that keeps it pointed at the right target.
Sessions are weekly for the first two months to build a foundation, then frequency is reassessed with you. You set the pace, and you share only what you are comfortable sharing.

Is high-functioning depression a real diagnosis?
High-functioning depression is a descriptive term, not an official diagnosis, but it often maps onto persistent depressive disorder (dysthymia), a recognized long-lasting form of depression. The low mood underneath is real and treatable whatever we call it.
If I am still functioning, is it really depression?
Functioning and feeling well are not the same. Many people carry real, persistent low mood while meeting their responsibilities. Continuing to function does not mean you are not struggling, and it does not mean you should not get support.
Do I need a diagnosis to start?
No. If you have been living with low mood or emptiness, even while functioning, that is enough reason to book.
